What counts as manual handling

Manual handling means transporting or supporting a load by hand or bodily force - lifting, lowering, pushing, pulling, carrying or moving. A 'load' can be a box, a person, or an animal. Most back injuries at work come from manual handling done badly, not from one dramatic accident.

The law behind it

The Manual Handling Operations Regulations 1992 (MHOR), as amended in 2002, are the main law in Great Britain. They sit under the Health and Safety at Work etc. Act 1974. MHOR applies to almost every workplace - offices, warehouses, care homes, construction sites.

The employer's duties (in order)

  • Avoid hazardous manual handling where reasonably practicable - redesign the task, use a machine instead.
  • Assess the risk of any hazardous handling that cannot be avoided, using a proper risk assessment.
  • Reduce the risk of injury as far as reasonably practicable - this might mean training, team lifts, or handling aids.

This is the hierarchy examiners expect: avoid, then assess, then reduce. You cannot skip straight to 'reduce'.

The employee's duties

Workers must follow the systems of work set up for their safety, use equipment properly, and report anything unsafe. Manual handling is a two-way duty - the employer must provide a safe system, but the worker must actually use it.

TILE / TILEO - the assessment factors

MHOR requires assessment across four (sometimes five) linked factors:

  • Task - twisting, stooping, long carrying distances, repetitive movement.
  • Individual - the person's strength, health, pregnancy, training, whether they need extra help.
  • Load - weight, size, shape, whether it is stable, hot, or has sharp edges.
  • Environment - space, floor surface, lighting, temperature, obstructions.

Some versions add a fifth O for Other factors, such as PPE restricting movement.

The famous 'guideline weights'

HSE publishes guideline figures (not legal limits) shown as a diagram with a person split into shoulder, elbow, knuckle and mid-lower-leg zones. Roughly: 25kg for men and 16kg for women at the best lifting height (elbow height), close to the body. These figures drop as the load moves higher or lower, or further from the body, and drop further with twisting. They are a starting point for assessment, not a pass/fail limit.

Common mistakes to avoid in the exam

  • Thinking MHOR sets a strict maximum weight - it does not; the guideline figures are indicative only.

Why the spine is the focus

The spine is the part of the body most often damaged by poor manual handling. Back pain and disc injuries are the single biggest cause of manual handling related absence in UK workplaces, so exam questions on this topic come up a lot.

How the spine is built

The spine is a stack of 33 vertebrae (bones) separated by soft, gel filled discs that act as shock absorbers. It has natural curves that give it strength. Manual handling injuries usually happen because a task forces the spine out of its natural curve, or because sudden or repeated load overwhelms the discs and muscles.

The main injury types

  • Muscle strain: overstretching or tearing muscles that support the spine, often from sudden effort.
  • Ligament sprain: overstretching the tissue that connects vertebrae, common with twisting.
  • Disc prolapse ('slipped disc'): the soft centre of a disc pushes out and can press on nerves, causing sharp pain that may travel down the leg (sciatica).
  • Cumulative injury: damage that builds up slowly over months or years from repeated poor lifts, not one single event.

The moves that cause the most damage

  • Twisting the spine while it is loaded (bending and turning at the same time).

Rules that protect the spine

Common mistakes

  • Assuming a strong back means you cannot be injured. Even fit, strong people get disc and muscle injuries from bad technique.

    Risk assessment — TILE/LITE

    Why risk assessment comes first

    Under the Manual Handling Operations Regulations 1992 (as amended), the hierarchy is: avoid the hazardous manual handling operation so far as reasonably practicable; if it cannot be avoided, assess the risk; then reduce the risk so far as reasonably practicable. TILE is the memory tool used to structure that assessment.

    TILE breakdown

    • T - Task: does it involve twisting, stooping, reaching up or out, excessive lifting or lowering distances, repetitive or prolonged effort, or insufficient rest/recovery time between lifts?
    • I - Individual: does the person doing the task need unusual strength or height, have a health condition or injury, be pregnant, or lack training or experience for this specific job?
    • L - Load: is it heavy, bulky, unwieldy, hard to grip, unstable, sharp, hot, or does its contents shift (e.g. liquid, loose parts)?
    • E - Environment: are there space constraints, poor lighting, uneven or slippery floors, steps, extremes of temperature, or gusty/outdoor conditions?

    LITE - the same idea, different letter order

    Some training providers teach LITE instead of TILE - Load, Individual, Task, Environment. Both cover exactly the same four factors; only the order of the letters changes, so do not be thrown if a question uses LITE.

    Guideline weights are NOT limits

    HSE guideline figures (from the well-known lifting/lowering diagram) are 25kg for men and 16kg for women at waist height, close to the body, reducing as the load moves further from the body or above shoulder/below knee height. These are only a starting point for assessment - they assume ideal conditions, are reduced by twisting, and are not legal maximums. A lighter load in a bad position (twisted, one-handed, awkward grip) can be riskier than a heavier load lifted correctly.

    Pushing, pulling and carrying

    Manual handling is not just lifting. Pushing, pulling and carrying carry their own risks and their own rules, and exams love to test the differences.

    Pushing and pulling

    • Pushing is safer on the back than pulling wherever you have a choice, because pushing lets you use body weight and keeps the spine in a stronger position.
    • Always try to push a load rather than pull it.
    • Keep your feet well away from the load so you do not trip or stub your toes on it.
    • Keep your back straight and lean into the movement using your body weight, not just your arms.
    • Handles should sit between waist and shoulder height for the best control and the least strain.
    • Get the load moving smoothly rather than jerking it. It takes more force to start a load moving than to keep it moving, so a slow steady start is safer than a sudden shove.
    • If a trolley or truck is used, check the wheels turn freely. Sticking or wobbling wheels massively increase the effort needed.
    • On slopes, extra care is needed. Going downhill a load can run away from you; going uphill it can roll back onto you.

    Carrying

    • Keep the load as close to the body as possible. The further away a load is held, the greater the strain on the back, even for lighter items.
    • Keep the heaviest side of the load next to the body if the weight is not evenly balanced.
    • Do not twist while carrying. Turn using the feet, not the spine.
    • Make sure the route is clear before you start. Check for obstructions, uneven flooring, and that the destination is ready to receive the load.
    • Never carry a load so bulky it blocks your view of where you are walking.
